Please read the information below with regards to Medical Conditions. If you have a
child who already attends St. John Henry Newman and requires medication to be
given to the school, please ensure the school has current (not expired)
paraphernalia. For new students or new diagnoses, if one of these conditions
pertain to your child, please access the appropriate forms attached and see below for additional
information.
Reports of Students with Medical Conditions
To promote the safety and well-being of students with
prevalent medical conditions (Anaphylaxis,
Asthma, Diabetes, and/or Epilepsy), parents and/or guardians are
encouraged to confirm annually to the Principal or Principal's designate if a
student's medical status has changed by
completing and submitting appropriate forms. These forms are available in the Main
Office and are posted on the school website.
Please feel free to forward any questions and/or concerns to the
school administrative team.

prevalent medical conditions – Diabetes,
parents and/or guardians are encouraged to confirm annually to the Principal or
